Participants, families, Support Coordinators, clinicians, hospitals, advocates, and justice professionals are welcome to make a referral.
We promptly review all referrals, check funding requirements, and reach out to discuss immediate priorities and next steps.
We ensure participant consent is obtained and that the individual remains in full control of their goals, supports, and care team.
